Thank you for choosing clearPath. Before we can get your account set up, we need a bit of information from you. Please fill in the tables below as completely as you can — the more detail you can give us up front, the less back-and-forth we will need later.

If any of the fields do not apply to your organisation, leave them blank and we will follow up during the setup call. For questions about any part of this form, email sales@clearpathhealthsolutions.com.

Contact List

Please provide a list of contacts in your organization.

clearPath User Accounts

Please provide a list of user accounts that will need to access clearPath, additional account can be added at any time.

The Admin and Observer checkboxes are to indicate the role of the user. If you have a group of users that only audit they should be set to Observer.

Note

While the number of observer or auditor accounts do not affect the pricing of clearPath the number of Admin accounts do affect the price. The minimum number of Admin accounts is one per site.

Health Care Provider Types

clearPath includes the standard fifteen HCP types as outlined by the Just Clean Your Hands Program for both Acute Care and Long Term Care Facilities. If your organization uses any custom HCP types please include them in the table below.

Facilities and Patient Care Areas

Please provide a list of all the facilities and patient care areas in your organization. To save space you can put multiple PCA’s on the same line separated by a comma.

Importing Legacy Data

If you have legacy hand hygiene compliance that you would like to import into clearPath, please provide some detail as to the data. We can import data from a wide array of sources. We support Excel, XML, CSV data formats. We can import data from almost any data format.
